The State provides 5 Sick/Personal days per year. The District (Local) provides 2 Sick/Personal days per year. Your accumulated sick/personal days are noted on each paystub. Contact Donna Hamilton at ext 209 for further information.

This information is used to determine whether the absence qualifies as Family Medical Leave as defined by the FML Act of 1993. If State Sick/Personal Leave is selected, please note in comment box whether leave is the *result of a work related illness or injury.
