These pages are available to print out and fill in with your student and doctor's information. These are kept in the nurse's office.

Medications to Keep at School Less Than 15 Days

This page is available to print out and fill in with details for long term administration of student medications. This requires a doctor's signature.

Policy for Adminstering Medications

This page lists the policies concerning administration of medications at school.

Please keep the school nurse updated on new medical issues or concerns regarding your child, especially new allergies or medical conditions that are diagnosed. If your child becomes ill or injured at school, it is very important that we are able to contact someone, so please keep the school updated with home, work, and cell phone numbers, plus the number of a responsible adult we can call in the event we are unable to reach a parent.
